If you have been invited to this event you must complete the registration form to secure your details.

The Foundation recommends that you make your travel arrangements as soon as possible. Please arrange your travel so you can attend the entire meeting. For direct billed travel, contact Diane Gibson when you are ready to make your travel arrangements.

Your room will be reserved for you based on your travel details.

Airfare, lodging, and on-site meals are paid directly by JSMF. You are responsible for submitting itemized receipts for reimbursement of other travel expenses within 30 days of the close of the workshop using the reimbursement form. Reimbursement will occur 4-8 weeks following the close of the meeting.
